In line with the “Vision Assessment” section of the DVLA D4 form (click there to see the actual DVLA page):

• If you DO NOT wear corrective lenses (glasses, contact lenses or both) for driving, your vision will be assessed during the medical by Dr Wilson, in line with D4 requirements.

• If you DO wear corrective lenses (glasses, contact lenses or both) for driving:

  • If you wear GLASSES ONLY to drive, Dr Wilson will require you to bring your current driving glasses, and the optician’s prescription for them, to the medical.  Your vision will then be assessed in line with the D4 form for glasses wearers.

  • If you drive while wearing CONTACT LENSES ONLY, please bring your current driving contact lenses, so your vision can be tested both with the contact lenses in and out – providing the contact lenses are “well tolerated”, that is all that is required; no prescription.  If you might struggle to put your contact lenses back in afterwards, as you have no glasses to wear, please ensure you have someone to drive you to and from the medical.

  • If you drive while wearing CONTACT LENSES and/or GLASSES, you will be considered as a wearer of both and as such will be required to bring all driving related items (glasses, contact lenses and any related prescriptions) to the medical.

 

For any of the above, if you meet the D4 requirements, Dr Wilson will complete the “Vision Assessment” section of the D4 form.

Should you FAIL to meet the requirements, you will need to consult an optician afterwards who will then need to complete the “Vision Assessment” section of the form – you will not need to return to Dr Wilson.
